The Oregon Health Plan gained national attention by changing the focus of health care from who is covered to what is covered. This change was facilitated by insurance reforms in the areas of small market, employer mandates, high risk pooling and Medicaid. Most controversial of the reforms is the use by the legislature of a prioritized list of health services to determine benefit levels for the insurance programs. Significant debate has occured over whether the use of such a list is (...) rationing or reasoning. The Oregon Health Plan represents a thoughtful and deliberate blending of fact with public value for the purpose of responsible health policy. It is that unique blending of public values developed through community participation with fact that focused the attention of the world on Oregon. Within many approaches to the interpretation of quantum mechanics, especially modal interpretations, one singles out a particular decomposition of the state vector in order to fix the properties that are well-defined for the system. We present a novel proposal for this preferred decomposition. Given a distinguished factorization of the Hilbert space, it is the decomposition that minimizes the Ingarden–Urbanik entropy from among all product decompositions with respect to the distinguished factorization. We incorporate this choice of preferred decomposition into a framework (...) for modal interpretations and investigate in detail the extent to which it provides a solution to the measurement problem and the extent to which it ensures that measurements whose outcomes are predictable with probability 1 reveal pre-existing properties of the system under investigation. Modal interpretations constitute a particular approach to associating dynamical variables with physical systems in quantum mechanics. Given the “quantum logical” constraints that are typically adopted by such interpretations, only certain sets of variables can be taken to be simultaneously definite-valued, and only certain sets of values can be ascribed to these variables at a given time. Moreover, each allowable set of variables and values can be uniquely specified by a single “core” projector in the Hilbert space associated with the system. (...) In general, the core projector can be one of several possibilities at a given time. In most previous modal interpretations, the different possible core projectors have formed an orthogonal set. This paper investigates the possibility of adopting a non-orthogonal set. It is demonstrated that such non-orthogonality is required if measurements for which the outcome can be predicted with probability 1 are to reveal the pre-existing value of the variable measured, an assumption which has traditionally constituted a strong motivation for the modal approach. The fourteen‐day limit was adopted as a compromise but subsequently has been considered an ethical line. Does it remain relevant in light of technological advances permitting embryo maturation beyond it? Should it be changed and, if so, how and why? What justifications would be necessary to expand the limit, particularly given that (...) doing so would violate some people's moral commitments regarding human embryos? Robust stakeholder engagement preceded adoption of the fourteen‐day limit and should arguably be part of efforts to reassess it. Such engagement could also consider the need for enhanced oversight of human embryo research. In this consensus report by a diverse group of academics who conduct and/or are concerned about social and behavioral genomics (SBG) research, the authors recount the often‐ugly history of scientific attempts to understand the genetic contributions to human behaviors and social outcomes. They then describe what the current science—including genomewide association studies and polygenic indexes—can and cannot tell us, as well as its risks and potential benefits. They conclude with a discussion of responsible behavior in the context of SBG research. (...) SBG research that compares individuals within a group according to a “sensitive” phenotype requires extra attention to responsible conduct and to responsible communication about the research and its findings. SBG research (1) on sensitive phenotypes that (2) compares two or more groups defined by (a) race, (b) ethnicity, or (c) genetic ancestry (where genetic ancestry could easily be misunderstood as race or ethnicity) requires a compelling justification to be conducted, funded, or published. All authors agree that this justification at least requires a convincing argument that a study's design could yield scientifically valid results; some authors would additionally require the study to have a socially favorable risk‐benefit profile. On the lives and philosophy of the Prophets of Islam and Sufi Muslim saints.

Despite its effectiveness, limited research has examined the provision of telemental health and how practices may vary according to treatment paradigm. We surveyed 276 community mental health providers registered with a commercial telemedicine platform. Most providers reported primarily offering TMH services to adults with anxiety, depression, and trauma-and stressor-related disorders in individual therapy formats. Approximately 82% of TMH providers reported endorsing the use of Cognitive Behavioral Therapy in their remote practice. The most commonly used in-session and between-session exercises included coping (...) and emotion regulation, problem solving, mindfulness, interpersonal skills, relaxation, and modifying and addressing core beliefs. CBT TMH providers had a higher odds of using in-session and homework exercises and assigning them through postal mail, email or fax methods, as compared to non-CBT TMH providers. TMH providers, regardless of treatment paradigm, felt that assigning homework was neither easy nor difficult and they believed their patients were somewhat-to-moderately compliant to their assigned exercises. CBT TMH providers also collected clinical information from their patients more often than non-CBT TMH providers. They reported being less satisfied with their method, which was identified most often as paper-based surveys and forms. Overall, TMH providers employ evidence-based treatments to their patients remotely, with CBT TMH providers most likely to do so. Findings highlight the need for innovative solutions to improve how TMH providers that endorse following the CBT treatment paradigm remotely assign homework and collect clinical data to increase their satisfaction via telemedicine. To date, our knowledge of Rift Valley fever disease spread and maintenance is still limited, as flooding, humid weather and presence of biting insects such as mosquitoes, have not completely explained RVF outbreaks. We propose a model that includes livestock, mosquitoes and ticks compartments structured according to their questing and feeding behaviour in order to study the possible role of ticks on the dynamics of RVF. To quantify disease transmission at the initial stage of the epidemic, we derive an explicit (...) formula of the basic reproductive number, $$R_0$$ R 0. Using the concept of Metzler matrix, we state necessary conditions for global asymptotic stability of the disease-free equilibrium. Results suggest that although host-ticks interactions may serve as disease reservoirs or disease amplifiers, the Aedes reproductive number should be kept under unity if disease post-epizootics activities are to be controlled. Results of both local and global sensitivity analysis of selected model parameters indicate that $$R_0$$ R 0 is more sensitive to the ticks attachment and detachment rates, probability of transmission from ticks to host and from host to ticks, length of infection in livestock and ticks death rate. Furthermore, when comparing the mean value of $$R_0$$ R 0 with that from previous studies which did not include ticks we found that our $$R_0$$ R 0 is very much larger resulting in an increase in the exponential phase of an outbreak. These findings suggest that if ticks are capable of transmitting the virus, they may be contributing to disease outbreaks and endemicity. (shrink)

People use words and concepts to refer to things. There are agents who refer, there are acts of referring, and there are tools to refer with: words and concepts. Reference is a relation between people and things, and also between words or concepts and things, and perhaps it involves all three things at once. It is not just any relation between an action or word and a thing; the list of things which can refer, people, words and concepts, is probably (...) not complete ; and a complete account would need to speak of cases in which the reference relation seems to involve three terms in a different way from the one already mentioned. In the philosophy of language, it has been customary to think of reference as a two-place relation, with some object as the second term and a word or phrase as the first. (shrink)

I shall describe yet another problem about fiction, similar in some respects to the ‘paradoxes of fiction’ on which so much ink has been spilt over the last quarter of a century. Since fictions are ‘made up’, what considerations stop us from making up our own endings to a fiction which is incomplete or whose ending we have lost or missed or whose ending is unpalatable?

The Primacy of God, the notion of justice toward God is seldom considered and often foreign. Far more discussed is how God might either undermine or motivate social justice. The Primacy of God by R. Jared Staudt offers an important intervention. With the aid of St. Thomas Aquinas, Staudt argues that it is vital for both contemporary society and contemporary Catholic theology to return to the traditional view of God as the one to whom all human and social action must (...) be ordered and to recover the virtue of religion as the virtue which orders all other virtues to God. Not only does Staudt helpfully remind readers of the ancient philosophical and biblical notion of worship as a dictate of the natural law, he also illuminates the way in which Christian liturgy, as an enactment of Christ's high priesthood, is the great fulfillment of natural and biblical worship. Accordingly, Staudt secures religion as essential for the virtue of love. This brings Staudt to criticize modern theologians like Karl Barth, who claimed that religion is inherently idolatrous, as well as Karl Rahner, who claimed that love of neighbor is the highest moral act. Staudt also considers the question of religious truth in light of the plurality of religions, soliciting the assistance of Hans Urs von Balthasar and Joseph Ratzinger, as well as the way in which religion relates to the development of culture, engaging the great Catholic social historian Christopher Dawson. The Primacy of God is a much-needed work that ought to set the agenda for Catholic theology in the twenty-first century. (shrink)

Allen examines Polanyi's and Hayek's thinking with respect to the nature, value, and foundations of liberty. For Allen, only Christianity, and certainly no modern philosophy, has a conception of the unique individual and his irreplaceable value and of a political order that transcends itself into the moral order. Beyond Liberalism challenges deeply ingrained notions of liberty and its meaning in modern society.

Among Plato's later dialogues, the _Parmenides_ is one of the most significant. Not only a document of profound philosophical importance in its own right, it also contributes to the understanding of Platonic dialogues that followed it, and it exhibits the foundations of the physics and ontology that Aristotle offered in his _Physics_ and _Metaphysics _VII. In this book, R.E. Allen provides a superb translation of the _Parmenides_ along with a structural analysis that procedes on the assumption that formal elements, logical (...) and dramatic, are important to its interpretation and that the argument of the _Parmenides_ is aporetic, a statement of metaphysical perplexities. Allen's original translation of and commentary on the _Parmenides_ were published in 1983 to great acclaim and have now been revised by the author. (shrink)
